Dog Adoption Enquiry Form

Thank you for your interest in adopting one of our dogs. To allow us to process your enquiry, please complete the form below. By answering each question in as much detail as possible, it will assist with your application. Please only apply when you are ready to take on a dog within the next two to three weeks. PLEASE NOTE THAT ALL VISITS TO THE ARK ARE BY PRIOR APPOINTMENT.

  • Please state the name of the dog OR the kind of dog you would like? E.g. Breed type/size, age, sex. We usually keep forms for up to 4 weeks, in case a suitable match becomes available.
  • This is to verify that the person applying is over 18yrs of age.
    We will need to see written proof.
  • Please give details.
  • Please list other adults living with you and their relationship to you.
  • Please list the number of children in your home and their ages.
  • If yes, please state their ages and frequency of visits.
  • Please list all dogs and include their age, gender, size/breed, if they are neutered/spayed and whether they are used to other dogs. Or not applicable
  • Please state how often they visit, and details of the visiting dog.
  • On an average day/week? Don't forget to include travel time to and from work.
    E.g. Friend or neighbour, paid dog walker or professional trainer.
  • For example, have you owned a dog before? Did you own it from a puppy or was it a rescue dog? Did you take on a dog with behavioural issues?
  • Please state approximate size of the garden, height of fences etc.
  • If you are planning to go away in the near future it might be worth considering delaying your enquiry until after your holiday.

Whilst every care is taken to find the perfect dog, we can’t guarantee behaviour, but may be able to offer behavioural support.

If you haven’t heard anything back from us within 4 weeks, assume that we haven’t found the right match for you. If you find your new companion elsewhere during this time, please let us know so we can remove you from our records.
